Kidney function and complications in patients with type 2 diabetes mellitus treated at the Jipijapa day hospital of the Ecuadorian institute of social security, January to July 2023

Type 2 diabetes mellitus is a pathology of heterogeneous etiology with an estimated population of 439 million worldwide by 2030. This disease has been related to microvascular complications, especially diabetic kidney disease, being the main cause of premature mortality. It is essential to diagnose patients who are more sensitive to developing kidney complications for better control of the disease process. The objective was to establish the relationship between kidney function and complications in patients with type 2 diabetes mellitus in the city of Jipijapa, January-July 2023. An observational, cross-sectional and retrospective study was carried out. Rigorous ethical considerations were complied with. Renal function test results (urine creatinine and microalbuminuria) were recorded in 1657 patients with diabetes during the period. Creatinine concentrations were found to be altered in 1.5% of men and 2.4% of women, while 6.2% of patients had elevated microalbuminuria. The renal complications identified in patients with abnormal renal function tests were acute kidney injury (63.1%), diabetic nephropathy with microalbuminuria (22.3%) and chronic kidney disease (14.6%), with an association found between the study variables. These results allow us to conclude that diabetes and its complications continue to be a health problem in Jipijapa and more preventive actions are needed, and a comprehensive therapeutic and clinical approach to these patients.
