Postpartum hemorrhage is a very complex risk factor that affects maternal mortality worldwide. Tranexamic acid is an antifibrinolytic drug, which has been traditionally used to control different types of hemorrhage. Currently, it is emerging as a potential treatment to reduce blood loss during childbirth. A systematic review was conducted using PRISMA methodology, with inclusion and exclusion criteria, selecting only clinical trials. Two case studies were included given the relevance of their data. In general terms, it was shown that the use of tranexamic acid is a positive alternative for the treatment of postpartum hemorrhage. Studies reveal that hemorrhage control is efficient both during childbirth and cesarean section. Hemorrhage control is also important to prevent maternal deaths. The use of tranexamic acid combined with other treatments such as oxytocin and protocols such as EMOTIVE, contribute significantly to controlling and preventing postpartum hemorrhage.
