Resumen

La cirrosis hepática crónica y progresiva, que puede tener muchas causas diferentes, se distingue por una fibrosis excesiva del tejido hepático y cambios en la arquitectura lobulillar. La inflamación crónica, el estrés oxidativo y la disfunción mitocondrial están vinculados en un circuito de retroalimentación negativa que se desarrolla a medida que empeora la cirrosis, lo que eventualmente resulta en daño celular y muerte hepatocelular. El término "síndrome de disfunción multiorgánica en la cirrosis" (MODS) se creó como evidencia de la participación de órganos distintos del hígado en pacientes cirróticos. Debido a la liberación disfuncional de mediadores inflamatorios, endoteliales y de la coagulación por parte del hígado, la translocación bacteriana y la respuesta inmune, este síndrome implica una serie de cambios en los sistemas, incluidos los sistemas renal, cardiovascular, pulmonar y cerebral, entre otros. La morbilidad y la mortalidad de los pacientes con cirrosis se ven significativamente afectadas por la disfunción multiorgánica, que también afecta la calidad de vida, complica la evaluación del trasplante de hígado, aumenta el riesgo de cirugía y aumenta el riesgo quirúrgico. El manejo clínico y la calidad de vida de las personas con cirrosis mejorarán con una comprensión más profunda de las interacciones entre los diversos órganos y sistemas.

ABSTRACT

Chronic and progressive cirrhosis of the liver, which can have many different causes, is distinguished by excessive fibrosis of liver tissue and changes in lobular architecture. Chronic inflammation, oxidative stress, and mitochondrial dysfunction are linked in a negative feedback loop that develops as cirrhosis worsens, eventually resulting in cell damage and hepatocellular death. The term "multi-organ dysfunction syndrome in cirrhosis" (MODS) was created as evidence for the involvement of organs other than the liver in cirrhotic patients. Due to the dysfunctional release of inflammatory, endothelial, and coagulation mediators by the liver, bacterial translocation, and the immune response, this syndrome involves a number of changes in systems, including the renal, cardiovascular, pulmonary, and brain systems, among others. others. The morbidity and mortality of patients with cirrhosis are significantly affected by multi-organ dysfunction, which also affects quality of life, complicates liver transplant evaluation, increases the risk of surgery, and increases surgical risk. The clinical management and quality of life of people with cirrhosis will improve with a deeper understanding of the interactions between the various organs and systems.
