Proteinuria and casts in urinary sediment as indicators of kidney disease in patients attending the Ecuadorian Social Security Institute Hospital - Manta

Abstract

The study focused on proteinuria and urinary casts as key indicators for the detection and monitoring of kidney disease progression in patients treated at the Ecuadorian Social Security Institute Hospital in Manta. Its primary objective was to evaluate the clinical utility of these markers for the early identification of kidney disorders, thus enabling timely interventions and a better prognosis. The study was analytical, non-experimental, cross-sectional, and retrospective, classified as low-risk. The sample consisted of 3,000 patients aged 18 to 60 years, including 200 individuals who met the inclusion criteria. The results showed that 68% of patients had cellular casts, associated with chronic or advanced kidney disease. In contrast, hyaline casts accounted for 14% and granular casts 18%, which showed a low prevalence, which could be related to early stages of kidney damage. In conclusion, urinary sediment showed a low prevalence of hyaline and granular casts relative to proteinuria. The findings of this study reinforce the diagnostic value of this analysis as an accessible, noninvasive tool for identifying kidney damage.
