Interferencia analítica en inmunoensayos: impacto clínico y estrategias de mitigación

Los inmunoensayos son una metodología del laboratorio muy útil y esencial en el diagnóstico moderno para cuantificar hormonas, marcadores tumorales, biomarcadores cardíacos y pruebas infecciosas. Sin embargo, puede tener interferencias analíticas de origen endógeno y exógeno, lo que genera resultados falsamente elevados o disminuidos, repercutiendo en el diagnóstico del paciente. El objetivo de este estudio es analizar las interferencias en el análisis de inmunoensayos, el impacto en la interpretación de sus resultados, además de las estrategias disponibles para su mitigación. Por ello se realizó una revisión sistemática en base a la literatura disponible siguiendo las directrices PRISMA 2020. La búsqueda se realizó en diferentes fuentes de datos, donde se incluyeron estudios del año 2021 al 2026 acerca de las interferencias en inmunoensayos. Se extrajeron datos del tipo de interferencia, analito afectado, plataforma analítica, magnitud del error, consecuencia clínica y mitigación utilizada. En los resultados, de 380 registros identificados, se incluyeron 31 estudios. Los principales interferentes fueron anticuerpos heterófilos, autoanticuerpos, macroformas, biotina y fármacos. Los analíticos con mayor interferencia fueron la hormona paratiroidea, la hormona estimulante de tiroides, prolactina, troponina, entre otros, lo que impactó en endocrinología, cardiología, oncología, infectología y medicina reproductiva. Para concluir, las interferencias analíticas en los inmunoensayos son una causa importante de error en un diagnóstico, por lo que su detección requiere correlación en la parte clínica y de laboratorio, además de la aplicación de diluciones, bloqueados, polietilenglicol, suspensión de biótica y métodos de confirmación alternos.
